The Financial Counselor/Practice Manager, Kristin Feldkercher will meet/talk with you and help you determine your current coverage. We encourage all patients be informed and current with their individual health contracts.

COVERED SERVICES:
If we participate with your current insurance carrier and the service performed is a covered benefit, we will accept the amount your carrier deems reasonable or allowable. If our charge is greater than the reasonable amount determined by your insurance, as participating providers, we will reduce our charge to the reasonable amount. Your responsibility may be a co-payment (a fixed dollar amount per visit) or co-insurance (a percentage of the reasonable amount), and all deductibles must be paid at the time of service.

NON-COVERED SERVICES:
There may be services that are not covered by your insurance company, and you will be required to pay for those services when they are rendered. We will inform you of those services at the time of your visit.

Many Patients are currently enrolled in HMO's (Health Maintenance
Organization) or POS (point of service) plans. It is your responsibility to obtain the appropriate referral(s) for your initial visit and all follow up visits from your primary care doctor.

We do file all health insurance with the appropriate carriers, whether we participate or not.

FEES:
Our fees are comparable to those charged by other specialists in this area. The office manager will give you an itemized statement before you leave. We request that patients pay for office services the same day unless special arrangements have been made in advance with the office manager.

PAYMENT TYPES:
We accept personal checks, cash, MasterCard, Visa, Discover.

Any outstanding balances must be cleared prior to beginning treatment. Balances from any prior treatment cycles must also be cleared prior to beginning a new treatment cycle.
