New patients should allow 45-60 minutes depending on the complexity of their case. Our office will mail you a health questionnaire a few days before your appointment. It is extremely important to bring, in addition to the questionnaire, all of your medical records for review from your referring physician, including:
  • Record of history and physical exam.
  • Record of all laboratory tests.
  • Reports of thyroid ultrasound, biopsy, bone densitometry scans and blood sugar readings.

At the initial visit, Dr. Antunes or Jill Drachler, RN, MS, ARNP will complete a history relevant to your problem and perform a physical examination. After that, Dr. Antunes will discuss your diagnosis and treatment plan and order the proper tests. If you are diabetic, you will be referred to Eva Martin, who is the diabetes educator. After the results are back, you will receive a call and be informed of the results and what the next step will be. If you do not receive a call within one week please call our nurse at: 941-955-2543 and ask for the results of your tests.

If you have been referred by another doctor, a letter will be sent to him or her in a reasonable period of time.

The office will need the following information when booking a new
patient appointment: 

  • The patients legal first and last name, address, date of
    birth, social security number and name of referring doctor. 
  • We ask that the when a New Patient books an appointment to please provide insurance information, specifically the group number, the I.D. number and the post office address to mail claims.

Please contact your referring physician and request that they fax any
significant results that pertain to the reason for your appointment to: 941-955-3181.

Should you be a member of a Health Maintenance Organization, ie: Aetna
HMO, Varsity, Health Options, please have you authorization faxed from your Primary Care Physician.

You should receive an email from us confirming your appointment within 24 hours. If not, please give us a call at 365-0333.

If you cannot keep an appointment, please try to inform us as soon as possible. This courtesy will allow us to give that time to another patient.

Follow-up appointments will be scheduled as needed. If you are a diabetic, you will be requested to bring the record of your blood sugars and your glucose meter on that re-visit. We also recommend that you bring an up- dated list of all your medicines.
